Description

Are you a highly relational leader?  Do you consider yourself a team-player and a self-starter? The Refuge Clinic is looking for a full-time Patient and Community Advocate to be an advocate for the underserved.  If you have a genuine desire to see life change, are a great communicator and well organized, this may be the position for you!

Primary Responsibilities Include:

  1. Facilitate all referrals to medical specialists, social services, and mental health professionals; building a strong resource bank for Refuge patients by developing relationships within the medical and social agencies in Fayette and Jessamine County.

  2. Develop new educational and relational programs to meet patient needs accordingly. Help identify and break barriers that limit patients from receiving optimal medical, dental, and spiritual care (i.e. language barrier- recruit and assign translators)

  3. Facilitate referrals to Helping Through Him (HTH), and construct a manageable system with HTH staff/volunteers to ensure proper care to Refuge Clinic patients.

  4. Facilitate referrals to the Kentucky Prescription Assistance Program (KPAP) and assist patients and providers to ensure proper completion of paperwork, including maintaining a tracking system for all medications ordered.

  5. Be a knowledgeable source for Fayette and Jessamine Refuge patients in regards to KCHIP, Medicaid, and Medicare.  Facilitate Refuge patients eligible for government assisted insurance in the procedures to obtain appropriate aids.

  6. Coordinate with local colleges and universities to welcome practicum students and serve as site supervisor for students in the area of social work (and other areas of study, as appropriate).

    The schedule for this position is Monday-Friday during regular business hours with occasional evenings.
